The started was initiated in partnership with CDC, Provincial General Hospital Kisumu and KUAP Community Health Programme. The Community Health Programme was chosen to be a partner because of the Community Based aspect. Pregnant mothers together with their partners and children who meet the criteria are enrolled in the initiative and given ART for life. The lives of families are strengthened through this initiative.This service is ensuring the child’s future through prevention
This project implements Mother to Child Transmission Plus. as part of the Team with New Nyanza Provincial CDC and using Peer Educator in service delivery; train Peer Educators; carry out Needs Assessment of clients accessing Services; monitor the health status of families in Project; refer clients for relevant services to other KUAP Health Projects; and implement PMCT Services.
Project goal:
To prevent MTCT of HIV among infected women, care and treatment of HIV infected women/families, comprehensive care to ANC/CWC
Objectives
- To provide quality maternal child health care
- To increase home visits on PMTCT and immunization
- Referral and networking with other project and stakeholders
Other activities
- Immunization of children under five years old against immunizable diseases
- Assess screen diagnose and treatment of minor illness
- Issuance of safe delivery cards to pregnant mothers to deliver safely in Hospitals
- Comprehensive approach to ANC thus FANC (Focused Antenatal Care giver)
- Counseling and testing given as a routine standard of package
- Refer HIV positive women to have minimum ARV prophylaxis
- Follow up visits to defaulted children on immunization
- Follow up visits t assess clients on care support adherence and medication
